ABSTRACT

Objective: To evaluate the efficacy of neoadjuvant chemotherapy (NACT) in the treatment of locally advanced olfactory neuroblastoma (ONB), and to explore the factors related to the efficacy of NACT. Methods: A total of 25 patients with ONB who underwent NACT in Beijing TongRen Hospital from April 2017 to July 2022 were retrospectively analyzed. There were 16 males and 9 females, with an average age of 44.9 years (ranged 26-72 years). There were 22 cases of Kadish stage C and 3 cases of stage D. After multiple disciplinary team(MDT) discussion, all patients were treated sequentially with NACT-surgery-radiotherapy. Among them, 17 cases were treated with taxol, cis-platinum and etoposide (TEP), 4 cases with taxol, nedaplatin and ifosfamide (TPI), 3 cases with TP, while 1 case with EP. SPSS 25.0 software was used for statistical analysis, and survival analyses were calculated based on the Kaplan-Meier method. Results: The overall response rate of NACT was 32% (8/25). Subsequently, 21 patients underwent extended endoscopic surgery and 4 patients underwent combined cranial-nasal approach. Three patients with stage D disease underwent cervical lymph node dissection. All patients received postoperative radiotherapy. The mean follow-up time was 44.2 months (ranged 6-67 months). The 5-year overall survival rate was 100.0%, and the 5-year disease-free survival rates was 94.4%. Before NACT, Ki-67 index was 60% (50%, 90%), while Ki-67 index was 20% (3%, 30%) after chemotherapy [M (Q1, Q3)]. The change of Ki-67 before and after NACT was statistically significant (Z=-24.24, P<0.05). The effects of age, gender, history of surgery, Hyams grade, Ki-67 index and chemotherapy regimen to NACT were analyzed. Ki-67 index≥25% and high Hyams grade were related to the efficacy of NACT (all P<0.05). Conclusions: NACT could reduce Ki-67 index in ONBs. High Ki-67 index and Hyams grade are clinical indicators sensitive to the efficacy of NACT. NACT-surgery-radiotherapy is effective for patients with locally advanced ONB.

ABSTRACT

Objective:To systematically analyze the chemical constituents of Qizhi Jiangtang capsules by ultra performance liquid chromatography-quadrupole-electrostatic field orbital trap high resolution mass spectrometry (UPLC-QE-Orbitrap-MS). Method:Analysis was conducted on a ACQUITY UPLC HSS T3 column (2.1 mm×100 mm, 1.8 μm) with acetonitrile (A)-water (B) as the mobile phase for gradient elution (0-13 min, 1%-25%A; 13-21 min, 25%-35%A; 21-28 min, 35%-85%A; 28-30 min, 85%-100%A; 30-32 min, 100%-1%A). The flow rate was 0.2 mL·min<sup>-1</sup>, the column temperature was 30 ℃, and the volume of sample injection was 3 μL. Electrospray ionization (ESI) was used to collect data in the negative and positive ion modes with the scanning range of <italic>m</italic>/<italic>z</italic> 100-1 500. Meanwhile, a variety of MS analytic methods were used, including comparing with the information of control substances, self-built compounds database and literature references, diagnostic ion filtering, Compound Discoverer 3.0 software, for identification of the chemical components. Result:Based on the above strategy, a total of 52 compounds were identified in Qizhi Jiangtang capsules, and the sources of these compounds were identified. Amino acids were mainly derived from Hirudo, phenylpropanoids were derived from Astragali Radix and Rehmanniae Radix, iridoid glycosides were derived from Rehmanniae Radix, coumarins and triterpenes were derived from Astragali Radix, flavonoids were from Astragali Radix and Polygonati Rhizoma. Conclusion:The established UPLC-QE-Orbitrap-MS analytical method can comprehensively and rapidly analyze and identify of the chemical constituents in Qizhi Jiangtang capsules. Many of the ingredients have been proved by modern pharmacological studies to have the effect of improving related symptoms of diabetes and its complications, reflecting the characteristics of synergistic action of multiple components in Qizhi Jiangtang capsules. This study can provide reference for the further research on the pharmacodynamic material basis and the quality control of Qizhi Jiangtang capsules.

ABSTRACT

Objective::Ultra high performance liquid chromatography coupled with Q-Exactive Focus hybrid quadrupole and orbitrap high resolution mass spectrometer (UHPLC-Q-Orbitrap HRMS) was applied for identification of chemical constituents in Pimpinella thellungiana. Method::Chromatographic separation was performed on a WATERS BEH C18 column (2.1 mm ×50 mm, 1.7 μm). The mobile phase consisted of 0.1% formic acid aq (A) and acetonitrile (B) with a gradient elution. Mass spectral analysis were performed on Q-Exactive Focus hybrid quadrupole and orbitrap mass spectrometer. The mass spectrometer was connected to UHPLC instrument via an ESI interface. Samples were analyzed in negative ion mode by the full-scan-dd MS 2(data-dependent MS/MS) scanning mode. Then the constituents of P. thellungiana were identified by compared HRMS data with those of the standard compounds, MS cleavage mechanism and the related literatures. Result::Based on the characteristic mass data of accurate molecular weight and fragmentation ion information, 29 chemical constituents were identified including 19 chlorogenic acids, 7 flavonoids and 3 phenolic acid. Among them, the identified components except luteolin-7-O-β-D-glucuronopyranoside and apigenin-7-O-β-D-glucuronopyranoside were reported in P. thellungiana for the first time. Conclusion::The established UHPLC-Q-Orbitrap HRMS method can be used to identify the chemical constituents of P. thellungiana quickly and accurately, providing the scientific evidence for its quality evaluation and material basis research.

?-arbutin is biosynthesized by whole cell method with Xanthomona maltophilia BT-112.The conditions for cell biosynthesized ?-arbutin are investigated as follows:temperature,25℃;concentration of hydroquinone,30mmol/L;mol ratio of sucrose and hydroquinone,20∶1;time course of ?-arbutin biosynthesis,45 hours;rotational speed,160r/min;concentration of Xanthomona maltophilia BT-112,85g/L;concentration of K-2HPO-4-KH-2PO-4 buffer solution,25mmol/L;pH of K-2HPO-4-KH-2PO-4 buffer solution,8.0.Under the above optimal conditions,the maximum of molar conversion yield based on the amount of hydroquinone supplied reaches 86.7%.
